Here are all the available print sizes:
Size Escape code

9 CPI (Extended) ESC n
10 CPI (Pica) ESC N
12 CPI (Elite) ESC E
13.4 CPI (Semicondensed) ESC e
15 CPI (Condensed) ESC q
17 CPI (Ultracondensed) ESC Q
Proportional 2 (144 DPI) ESC p
Proportional 1 (160 DPI) ESC P
Additionally, control-N turns on double-width mode, which makes any of the
above sizes print twice as wide as normal. Control-O returns to normal.
The default print size when you first turn on the printer is controlled by
DIP switches SW1-6 and SW1-7. The settings are as follows:
SW1-6 SW1-7 Default print size
----- ----- ------------------
Open Open 10 CPI (Pica)
Closed Open 12 CPI (Elite)
Open Closed 17 CPI (Ultracondensed)
Closed Closed Proportional 1 (160 DPI)
The factory setting is "Closed, Open" (12 CPI).

Not that I'm aware of.
I think Apple still sells the Imagewriter II, in which case you might still
be able to buy a manual somewhere.
