AppleWorks GS has a page layout module that lets you combine text and
graphics (APF or AWGS native graphic formats).
GraphicWriter III v. 2.0 lets you combine text (import from text,
AppleWorks, AppleWriter, Teach and with modules from EGO Systems RTF and
AWGS) and graphics (DHR, SHR, MacPaint) even more easily.
I believe that the word processor that was the basis for the AppleWorks
GS word processor (from Styleware) allowed text and graphics to be
mixed. 
I think that BeagleWrite (sold by Scantron Quality) has an add on for
including graphics with text.
Also, there was the Newsroom, a DTP 8-bit program.

In addition, of course, to Publish It! and AppleWorks with TO.Paint as
Brian mentioned.
