Mitch Spector told me about this and gave me the part number of the card.
Turns out it's the regular ol' nubus 8 bit video card. I got one of these
in the $50 IIx I got for $50 from military surplus. The box was dead but
since it had the card plus two superdrives, it was worth getting.
I would expect this card to be fairly common since it's pretty standard.
Nubus cards in general and 8 bit video aren't much in demand these days
so ought to be pretty cheap too.

I tried the card in my 840AV. Boy, multiple monitor support is a really slick
feature. I can run telnet on a 13" monitor connected to the video card and
run Netscape etc on my 16" with 24 bit color on the built in port. I'd keep
both monitors connected all the time if I had the desk space. 8 bit video is
wicked fast on the 840 displaying text oriented apps.
I don't plan to try a IIgs monitor on the card so can't verify how well it
works. As a rule I don't connect stuff together just because I happened to
hear that they'll work.
