If you have the disk with the AA application on it, then you should have
all the software you need.  The AA comes with a second disk, but there's
nothing essential on it--just sample sound and MIDI files.

It sounds like the file you need is "CARD8530.MIDI".  You can find it on
the AA application disk, in the SYSTEM/DRIVERS directory.  You need to copy
it to the SYSTEM/DRIVERS directory of your boot disk, and then select it
using the MIDI CDEV.

You have to use the driver from the AA disk...the AA hardware isn't compatible
with either of Apple's drivers.
