It's Quark Catalyst. It is very similar to MouseDesk (aka Apple II Desktop).
I too have a damaged copy of Catalyst. Perhaps we can come up with a working
copy between the two of us!

Catalyst was packaged with the Unidisk 3.5 drive when it first came out back
in 84 or 85.
