I have found one.  Many older programs (including anything written in
runtime Pascal, such as the old PFS: suite), including my DOS 3.3 version
of AppleWriter ][e use a method of displaying text which chokes on the "new"
video ROM...where you are supposed to get inverse uppercase text, you get
Mouse$#!+ instead.

Apart from that, I can't think of any difficulties with the enhancement. 
If I were a bit more inventive, I'd come up with a switch to toggle
between the original and enhanced video ROM, and leave everything else
enhanced.
