Help: Versa Card

Hi There,
I just got a Versacard from a garage sale. I believe it is a 
serial/paralel/clock card for AppleII. It looks neat, however, there is no 
manual, no cable, and it seems 1 chip is missing.
Does anybody know what is the type of the chip? It is located at socket U12 
and has 12 pins.
There are two blocks of DIP switch (6 switches in each block). How to set 
these switches?
Could I plugged an IBM XT serial cable to it?

Any help would be appreciated.
