ALS- CP/M

Hello all,
    My Department recently wanted to reserect an old experiment that used an
apple2 with Digital Research's "The CP/M Card". I Can't seem to get the
thing to boot off the #1 floppy.
    Is there anyone that can e-mail me a dsk image of a bootable floppy for
this card?
