YKH-64 EPROM Programmer for Apple II



I have a EPROM Programmer for a Apple II that I am trying to figure out.
The only identifing marking on the card is S.M.033 and a cross.

It has a 28 pin ZIF socket and a DIP switch (8 position) for selecting
EPROM size that is labeled 16, 32, 64 (for 2716, 2732, 2764).
It has firmware on the card.
 
With the card in slot 2, if I enter PR#2 from the keyboard the following
appears.

?(I enter 2 and get)
 
1) WRITE

3) COPY
4) COMPARE

START ADDRESS ? $ ( I enter 0000)
 THE SCREEN FILLS WITH SOLID CURSOR BLOCKS AND THE COMPUTER LOCKS UP.
 
 
Can anyone help me?
I'd like to get the docs for this.

PS A weird chip is in the U1 socket on the card.
   It is labeled       820-0006-02
                       APPLE-P6AB3
 
Isn't that the PROM from a Disk ][ controller?
