Hayes Micromodem

If anyone has any information about the Hayes micromodem (300 baud), I need
to know th pin-out configuration for the telephone line jack plug on the card.

It's an internal card and came originally with a `proprietary' connector cable
that I do not have.  The jack appears to have the telephone line output 
interface and some other logic to the plug. ???

If someone knows what the pinout of the plug is, I'd appreciate the info.
