CMS SCSI ?

I picked up a CMS HD and SCSI card for my IIgs. Unfortunately,
the great deal I got did not include any manuals so I've
come to the net for advice.

1) What jumpers should be set on the SCSI Card?

2) What are the settings for the dip switches on
   the HD (A CMS 60 meg)?

3) I was told that the drive is internally terminated.
   Is this true? If not, where do I find a terminator
   that fits those small, non standard SCSI interfaces?
