That is the Apple High-speed SCSI card. The original card is dated 1986.

What disk would this be? If you are trying to use the software that
was supplied with the original Apple SCSI card, I'd be very surprised
if any of it worked with the high-speed card. Most of these files are
dated 1988 or earlier. The important ones:
BACKUPII (dated October 1985)
HDSCPARTITION (dated January 1988) - format and partition utility
STARTUP (dated January 1988) - SCSI verify utility
HD.SCSI.ASM.0 (dated August 1986) - used by STARTUP
There are other miscellaneous files on the disk (PRODOS, BASIC.SYSTEM,
LAUNCHER.SYSTEM).
The major files supplied on the disk with the High-Speed SCSI card are:
BACKUPII (dated March 1990)
SCSIVERIFY (dated February 1990)
HDSCPARTITION (dated February 1990)
This version of HDSCPARTITION will only work with the High-Speed SCSI
card. The older version will only work with the original SCSI card.
What computer do you have? I assume it is an enhanced IIe. If you
have a IIgs, you should be using GS/OS to set up the hard drive.
