Why not use Advanced Disk Utility which comes with the IIgs system software?
It can do everything that the 8 bit programs that come with the card can do.
Even more actually since you can format non ProDOS volumes with it.
Actually Chinook SCSI Utilities is the thing to have but it's out of print
now and I'm keeping mine. But even with it on hand I use ADU for formatting
and partitioning etc.
