The most readily available disassembly of Applesoft BASIC is the
SOURCEROR.FP program included with the Merlin 8/16 assembler, from
Roger Wagner Publishing.
I think "Apple Assembly Line" also did a full Applesoft disassembly,
in which case it might be available on GEnie (but only to GEnie
subscribers - you aren't allowed to distribute them elsewhere).

The Apple IIe Technical Reference is available from Resource Central,
for US$24.95 (extra if you want it air-mailed). It doesn't cover any
entry points in Applesoft BASIC ($D000-$F7FF), but does list and
explain all of the official entry points in the monitor ($F800-$FFFF)
and 80-column firmware ($C300-$C3FF). It also covers the IIe hardware
in great detail, and explains how to use the monitor.
