Well, the //e Technical Reference Manual is about your only source for the 
Monitor ROMs and $C0 ROMs listings, straight from Apple.  That has MOST of 
all of the Apple ROMs, except the Applesoft ROM listings, which I don't 
believe exist anywhere (legally) in published material.  However, there IS a 
book that I found in the UT library which was indispensable.  It is actually 
for the II/II+, but will help with the Applesoft ROMs.  It's called "What's
Where in the Apple II" and has a lot of useful info, but the most useful
part is in the back where they have an annotated list of all memory locations 
(not just ROM locations) with descriptions of each.  If you wanted to make
a listing of the Applesoft ROMs and then add the comments and labels to all
of these locations, you'd have all of the Applesoft entry locations for all 
sorts of things.  There are many more useful entry locations besides those 
documented in the Apple tech ref.  Only thing is that they may not be 
compatible with a laser, etc, where the Applesoft ROMs are clones..
