I'm not sure what could be causing this. However it might be because the
hex calculator is activated. Try clicking on the zoom box on the right
side of the calculator window bar to reveal all the calculator's
functions and select the DEC key.

Nothing is wrong. You've got GS.Spy init installed. In addition to
providing an "LED" drive activity display in the menubar, it also acts a
virus checker, and an extra precaution against accidental formatting.
When you get the 'ALERT ALERT' warning, press return to continue
formatting or esc to go back.
 
GS.Spy also prevents HardPressed from working, so if you have HardPressed
you should deactivate the GS.Spy init.
