Yes, it is a fault in one of the chips on the Disk ][ controller card.
But DONT discard your old controller card!  The fault is likely just one
bad chip, which can be identified by doing a sysematic tematic chip-swap (bad-
to-good) with a card that is known to be good.

If you mail the bad controller card to me I will do this.  I have a 
supplky of the ICs for the Disk II controller card and can check it out 
for you, and the cost would be just the postage and the cost of the 
replaced chip ($2 plus $2 = $4 total).  Alternatively, I would buy
your as-is Disk II card for $3 plus $2 postage = $5 total).
