There is a program called CrossWorks. It has two parts. First is
communications software and a serial cable for transferring files
to and from an Apple II and an MS-DOS computer. The second
converts Appleworks files (up to version 3) to various MS-DOS
formats.
Any Apple II word processor that can export plain ASCII files will
work. For bold, italic, and underline you can use generic markers
(like HTML <B>, <I>, and <U>) and edit them in the final formatting
pass.
