I have a MicroDrive with attached 2.5" HDD in S2 and a ComputerEyes video card in S3 with the Uther in S4, though I had my first Uther in that set-up for over a year and it's still fine. Maybe just bad luck?

All the power line filtering in the world won't save the PHY interface from events carried in on the ethernet cable itself. Do you live in an area with frequent electrical storms and/or lots of dry weather?

That was going to be my next line of defense. I never leave any external lines plugged into my Apple IIs when not in use. Basically, because the next one I'm setting up uses the same cables, most of the time. But, I would try swapping the modules between the two cards, as it could be just the 74xxx245, which is cheap and easy to replace. The main card is merely an interface between the Apple buss and the Ethernet module.
