Thanks guys, you have fixed my problem and increased my store of
knowledge.  It turns out that the Mouse card being in slot 3 was a bad
idea.  I moved it to slot 4 and the Mockingboard to slot 5 and Ultima
V shows the "Activate Music" option and plays it too.  Ultima III also
plays music just after the Mockingboard selection screen now instead
of nasty disk drive noises.  Finally my Apple IIe is more than just an
oversized paperweight.

Speaking of which, slot 3 was traditionally reserved for Z-80
softcards.  Do those cards still work in that slot?  If not, will the
software that supports it work with it in another slot?  Is there a
typical use for slot 3 in the IIe or should its use be avoided unless
absolutely necessary?
