The only difference is the ROMs and the 65C02 instead of a 6502A, but
the board you have there is definitely running unenhanced ROMS.
I'd just get the bits and upgrade it, unless it's a *really* early
Rev.A IIe board - those are collectable items.

It's almost certainly controller failure. The error mode you describe
is very common, I've seen it numerous times. I've never bothered to
diagnose exactly what is dead on the controller cards, since I've got
so many of them.
