The Commodore 1581 writes "bottom" MFM sector adresses on the "top"
side and vice versa, but it's of course a double-sided drive.

Never heard of a drive that used only the top side, but given the
assortment of odd critters created in the ca. 1977-1983 time frame, I'd
not be too surprised if they exist.

There weren't that many makers of the actual mechanisms, and most
of them simply imitated Shugart, so I'd be very surprised to see
a "standard" 5.25" drive with only a head on top.

Think about it--if there were such things, there never would have
been such a thing as a "single-sided floppy diskette" without being
system-specific.
