If you're coming from Windows, use something like AppleWin to create a 
startup floppy (can be ProDOS, but the disk ordering must be DOS) and use 
ADT_2004 to create the 140K floppy.  You can boot that and it will have 
whatever you setup on it.  Probably prodos, basic.system, copy ii plus, and 
shrinkit.  Add whatever else you want.

For ADT_2004, Google on ADT_2004.zip and you'll find a few hits.  Here an
