This might be an old thread by now... but I thought I'd pitch in:

Locksmith 5.0/6.0 Fast Copy, Apple II+ with Saturn 128K card.. Copy 140K disk in under 30 seconds.

This trick was done by reading sectors off the track as they came in... their decoding routines must have been rather clever and fast. Writing was done without formatting first, the entire track at once as far as I could tell.
