for an Apple IIe, look for 

341-0151-A  	IIe Keyboard ROM deutsch/usa
341-0161-A  	IIe Video ROM deutsch/usa

These should be available as binaries at the popular Apple II ftp
servers. 

The keyboard ROM contains the layout changes (swap Y and Z, put ; over ,
etc), the video rom contains the character bitmaps.
