I initially formatted them on an Amiga 3000 using a program
designed to format multiple disks. It verifies the disks after
formatting them. The ones that failed the verify test I then
formatted on a IIgs. The ones that failed the verify test on the
Amiga also were problems on the IIgs.

Why? The liner material was coming apart and is visible when you
expose the media surface. Major league junk.
