Interesting you have it in slot 4. Also I see you're using it in a
//gs.

Something to look for. I had a similar experience with a //e. The
problem turned out to be the mouse interrupt was causing the card to
crash. This stumped me for awhile. Until I noticed the thing worked
fine until I bumped the mouse. CRASH.
