I have come across many joysticks with more than two buttons on them
but they usually are just putting the same button in more than one
place for convenience. I've seen joysticks with two buttons on the
left and two buttons on the right but it was just button 0 and 1 on
each side so it could be used easily by either left or right handed
users.
I think I still have my Apple II Gravis like you pictured above and
I'm pretty sure the button on the stick is button 0 and the buttons on
the base are button 0 and 1 so it is another instance of extra buttons
being there for convenience.
