David, thanks for the offer, however the original seller sent me a new
card and all appears to be working as expected.

In summary:

I had a model A motherboard that apparently wouldn't have worked with
the card in the first place.  I updated the motherboard and also
gained an enhanced //e at the same time, but the card still didn't
work.  I received a replacement card and all is well.
