Sorry, can't help with the card.  But...

In theory at least, you can put up to seven SSCs in an Apple //e.  Of
course, if you do that you give up your disk drives and 80-column card.  8-)

The most I have ever seen in a //e was three...one for an ImageWriter, one
for an Apple Plotter (anyone else remember those?), and one for a modem. 
That machine was getting kind of crowded, as it also had a mouse card and
a disk controller, plus a special custom interface card for a Tasman
Turtle.  So that machine had...
Slot 1:  ImageWriter SSC
Slot 2:  Modem SSC
Slot 3:  80-column card (in Aux slot)
Slot 4:  Apple Mouse
Slot 5:  Apple Plotter SSC
Slot 6:  Disk Controller Card
Slot 7:  Tasman Turtle interface

That was a machine we were using at a computer education program I worked
for.  We had another machine that we used for making class sets of
software...it had eight or ten or twelve (whatever Penulti supported...)
Disk ]['s connected...
