This is how AppleWorks handles it. It has a specially constructed disk
which only has one block in the root directory (block 2), plus the
bitmap (block 3?). This frees up blocks 4, 5 and 6. (Alternatively,
the bitmap could remain in block 6, and blocks 3, 4 and 5 could be used
for data storage.)
SEG.M0 or SEG.M1 starts in this area. (ProDOS is quite happy to read
files here, but might have problems writing them.) SEG.PR ends up near
the end of the disk.
I had to reconstruct a damaged AppleWorks program disk a while ago. My
solution was to manually patch the root directory and index block for
SEG.M0, and manually copy three blocks from the file into the area
normally occupied by the directory and bitmap. (I did this using ZAP,
part of "Bag of Tricks 2", but it should be possible with any decent
block editor.)
