Most of your questions are answered by the comp.sys.apple2
Frequently Asked Questions (FAQ), which is at
http://www.visi.com/~nathan/a2/faq/csa2.html section 5. You'll
probably find that a null modem either used by a comm program or
'printing' to that port will do best. Except for the somewhat rare //e
emulation card, Macs have never supported Apple II 5.25" disks.
