Yes, I have one of these too and I can attest that it works beautifully.
The manufacturer is Opcode, if that helps.
Full name: Opcode MIDI Translator II.
You can even switch between MIDI and MODEM when the power is on!
The manual no longer mentions the IIGS, but the copyright notice attests
that previous versions did ("Macintosh and Apple IIGS are trademarks of
Apple Computer, Inc...")
