I wasn't aware of there being any hardware revisions - I have an
original card (shipped with Rev A ROM) which was later upgraded to a
Rev C ROM.

That's interesting - I have the same problem with my old Rodime 69 MB
drive. It works fine with the System 5.0.4 drivers (on either the
original or High Speed Apple SCSI cards), but doesn't work with the
6.0 or 6.0.1 drivers on the original Apple SCSI card (it is fine on
the High Speed card).
You may run into all sorts of nasty problems if you use the 5.0.4
drivers in System 6.0 or later - programs may assume that newer
features are available. In particular, do _not_ use the System 6
version of Advanced Disk Utility with the 5.0.4 drivers - use 5.0.4's
ADU instead. Don't try to mark partitions "read only" from Finder,
either.
