I had a similar problem a while ago, and it turned out that I had just
installed System 6 a day or so before, and so the file */system/system.setup/
Easy.Access was installed. Easy Access is still pretty buggy, so, apoligies
to those who actually use and need it, if you don't need it, just kill it.
You can test this out by first disabling Easy Access (from the finder click
once on the icon, and select Get Info from the menus. Then, click on the
"Disabled" box, and reboot. If your problems go away, you can kill Easy
Access.
