For me, it's:
- dread of customs paperwork for UPS or Fedex, unless carrying item to
Post Office.  Much easier there.

- sweating and stressing out the long delivery time, wondering if the
stuff will ever arrive there in one piece, still working, despite tank-
like packing.

- wondering what to do if buyer says it never arrives.

The major impediments are lack of tracking, and the costs of shipping
back and forth.  I've always done insurance with return receipt, and
never once have I received a return receipt, from any country.

So I perceive the process as very, very risk.. and unless the gain is
very, very high over what USA bidders will provide, I won't engage in
it.   Sometimes the gain HAS been much higher, and so I've taken the
risk.   On the positive side, it's been less than a dozen times for
me, and each one went smoothly despite the sweaty palms for the 4-12
week deliveries.
