Judging by the picture in the manual you just need a female DE-15 crimp
connector, a 16 pin female IDC and a length of 16 core ribbon cable.

Ignore my pervious post. While the Apple RGB monitor cable is
constructed using a crimp DA-15, the VGA cable uses a solder type
DE-15F.

I have metered the connections and they do not make perfect sense.
There appear to be a couple of errors in my cable.

I've adjusted your table slightly...

IDC-16F DE-15F VGA function
------------------------------------------
  1 3 blue
  2 4 (monitor ID 2)
  3 2 green
  4 7 green gnd
  5 1 red
  6 6 red gnd
  7 NC
  8 NC
  9 5 gnd
10 13 Hsync (or composite sync)
 11 11 (monitor ID 0)
 12 14 Vsync

I get the same wiring (except that my IDC 9 connects to DE-15 5),
and it doesn't make sense--particularly when compared with the
"straight-through" IIgs RGB monitor cable:

IDC-16F DA-15F IIgs function
-----------------------------------
   1 1 red gnd
   2 9 blue
   3 2 red
   4 10
   5 3 composite sync
   6 11 (audio from IIgs)
   7 4
   8 12
   9 5 green
  10 13 blue gnd
  11 6 green gnd
  12 14
  13 7 (-5v from IIgs)
  14 15
  15 8 (+12v from IIgs)
  16 (not connected)

Does Apple use a non-standard connector numbering, or what?
