It actually worked quite reliably as it was designed.

But the designers did not foresee so many power-hungry add-on cards
overloading the built-in power supply.  If you check the designed
power budget for a slot card, you will see that many popular cards
are well over that budget.

Even then, the system usually worked when it was new.  But over time,
as the line supply filter capacitors dry out and lose capacitance, the
power supply margins are reduced until supply regulation suffers and
intermittent problems result.

Most power supplies that once worked reliably in an overloaded
configuration would work reliably once more if the line supply
filter capacitors were replaced or upgraded.

Degraded capacitors on the power outputs in the supply (or on the
main board) can also be a problem, since reduced decoupling adds
to noise on the power busses.

(My main //e started to fail within seconds of booting once when
the 0.1uF decoupling capacitor adjacent to the 65C02 on the main
board had one of its leads broken from too much flexing.)
