Hi Harvey,
I think your Apple II+ hasn't enough memory: I have read from Prodos User's
Manual page xi:
"...The ProDos User's Disk will not work on an Apple II. This is because
Applesoft Basic must be in ROM (read-only memory) for the disk to work.
Also, to use ProDOS, your computer must have at least 64 kilobytes (K) of
random-access memory (RAM). Most Apple II computers have 48K or 64K RAM.
Some earlier models have 16K or 32K RAM."
