It can be any SCSI card.  In fact, it'll work much better with an
Apple II High Speed SCSI Card or even a RamFAST.

Depends on the connector on the CMS drive.  If it is a standard
Centronics-50 connector, then you'd want a DB25 to Cen-50
SCSI cable.  If it is a DB25 connector, then you'd want a DB25
at both ends cable.

Depends on the SCSI card.

You can get the software for all three SCSI cards from
the AOL Apple II Forum.  Use the following keyword
to get there:

aol://4344:1264.a2main.10029531.514525857

Use the keyword button/command and clip and paste
to above keyword into the resulting dialog box.  Don't
just put it into the URL bar.
