I got the chip  and the format software from Bredon directly from
Bredon when I purchased Prosel and other items. Th software is marked
Advanced Tech Services. I was led to believe that Bredon was ATS by
another party when I purchased it. I do remember the order came from
Georgia originally.

I modified the drive myself immediately after purchase.

The ATS EPROM chip states:

D4A/D4T
PRODOS
103816 B
FCP 1990

The card itself has the following ID:
-its orientation is component side up, slot connector facing me.

XEBEC (Lower right hand corner)
FCC ID: C7FFKL103916APPLE (Right Top)
ASSY : 103916 05 - the 05 is taped on - Center Top
REV: H  05 -The 05 is taped on - Center Top toward right
S/N: 7-0707 - Right Top

The card is laid out in a very organized fashion.
(From the left)

50 pin Ribbon connector.
Primary Capacitor Bank (Total capacitors C1-C36)

C31 exists at the lower right of the card as a single capacitor.
Jumpers W1 and W2 (Intermixed but side by side)

Choke Bank: L1-L16

Followed by Chips: U1-U16.

Ic chips are laid out essentially in columns (Some a little diagonal).

Column 1 - 5 chips (U1, U5, RN1, U11, U14)
Column 2 - 4 chips (U4,U8,U12,U15)
Column 3 - 4 chips  (U2, U6 [the eprom], U10, U16
Column 4 - 4 chips ( U3,U7,U9,U13)

A Single resistor Network IC exists at RN1 marked CTS8651. It
markings/ID are upside down from the other chips.
