You mean Option and Control (Option to present the choose-application 
dialog, Control to make the association permanent).  This does not work 
with a "generic" document (one that does not have a special icon), which 
makes the feature slightly less useful than it could be.  You will have 
to use an icon or resource editor to assign an application to such a 
document.
