The problem could be coming from a lot of places, but I'd say that the most likely
cause is a flaky RAM chip, and second most likely being a flaky ROM chip. If you
can lay your hands on a copy of the Apple ][+ Dealer Diagnostics diskette, you
could probably nail the culprit in a couple of debugging sessions.
However, given the current low prices on Apple ][+ motherboards (see
comp.sys.apple2.marketplace for some examples), it might be more prudent to
simply swap your motherboard for another, and hold on to your current
motherboard as a source of spare parts.
