I have no certain answer, but my guesses are either HD corruption
that hit GS/OS, a misbehaving init/da that only shows up once in a
blue moon, or somthing else. If it was HD corruption, than
reinstalling stuff would fix it. If it only happened rarely (such as
running some sequence of programs that you're not likely to repeat),
then reinstalling wouldn't do much.
If it reoccurs and you can track it down for certain, then you could
truly know what it means.
