I don't see any evidence of a spurious fork on either the zip or the 
unzipped image. unzip -l shows a single file of the correct size:

$ unzip -l Cinemaware_Demo.zip              
Archive:  Cinemaware_Demo.zip
  Length     Date   Time    Name
 --------    ----   ----    ----
   819264  06-21-99 04:42   Cinemaware Demo.2mg
