This happened to me, and is a long shot, but worth checking into.
To the left of slot "0" toward the power supply there are several
IC's, one of which is a 1 to 8 decoder, a 74LS138. When I had slot
problems on one of my II+'s, this chip had 2 of the output pins
lifted out of the socket. Once they were straightened and thusly
reinserted into the socket, all of the slots worked properly.
