The one I have says:

APPLESOFT II
(c) 1977 MICROSOFT
P/N A2T0004X

Under the apple it says:

002-0007-00

It says "apple computer inc." in the Apple font, but it does not show
a copyright statement.

The back has the "Floating Point BASIC Demo (APPLESOFT Loaded)".
