Forgot about Disk2FDI! It's even in the FAQs ("Diskettes" page).
Decided to give it a try. The docs that come with the program are pretty
good; but, the collected Csa2 info was a huge help. It's referenced in the
FAQs:
http://apple2.org.za/gswv/a2zine/Docs/Disk2FDI_InfoPostings.txt .
Since I'm running Win/Me, I tried running Disk2FDI in a DOS window and
also under booted DOS 6.22. Seems to work as advertised both ways.

Disk2FDI works great with quite a few diskettes and not so well with
others. The latter include unprotected diskettes which work fine on our II+ and
Verify as having no errors under Copy II Plus.
I can tell you your experience is very similar to mine on all counts. Works under Windows 3.1 through 98/ME. Works under DOS. Running from floppy is a good option unless you are converting a bunch of disks due to storage space limitations; you can usually only get about 3-4 images on the floppy before it is full.
The disk2FDI program is pretty good but not perfect. I have found that even a good disk that was read without errors has an occasional bad byte or two.
Disk2FDI is slick if you need a data file moved over but I do not recommend it if you have an alternate means available when transferring an entire disk, it is not as accurate as using A2 hardware.
Any disk that is fully functional in your II+ may work in disk2FDI if you copy it to a fresh disk using a track copier... Locksmith Fast Copy is my preferred program. I believe this will eliminate those glitches and timing snafus that are introduced by sectors being written at a different times and allow the extrapolation process more accuracy.
