I very much doubt this is a modem. I think it is an interface for
an external keyboard of some sort.

This is an 80 column card - looks like a Videx-80 clone.
Stick it in slot 3, attach a length of coax cable to the two pins
in the upper left of your picture [not sure which is ground and which is
video out - see if you can follow the PCB traces to work it out) and
connect it to the video input on a monitor with 14MHz or higher bandwidth.
PR#3 at Applesoft prompt will enable it.
