You would end up having to patch or probably rewrite the AppleDisk5.25
driver to get it to support disk sensing with 5.25" drives so you
wouldn't necessarily have to get the signal back to the SmartPort,
although that would be the nicest design.  You could just hook up the
sensing circuit to the internal joystick connector.
